Most public Fediverse servers are running on professional hosting services using business-grade data centres. They are owned and controlled by grassroots admins, but they are on the same kind of hardware as commercial online services.
For some reason there's a misconception floating around that all Fediverse servers run on home machines in a corner of someone's living room. This usually isn't the case. It is possible to do, but most admins don't do this, especially not for public servers.

You don't have to use Mastodon through an app. You can access your Mastodon server through a web browser on computers, phones or tablets. Use the same account and password you use for your app.

Many settings and features are on the website but not on apps. If there's a feature or setting missing on your app, try seeing if it's there on the website.

Every account on here has a unique Fediverse address which looks like this:
@ (username) @ (server name)
You can share your Fedi address with others in real life the same way you share an email address or phone number.
These can be useful if someone's account isn't showing in normal searches on your server. You can paste an account's Fedi address into the search box in Mastodon and the account will appear.

Anyone can create their own fully independent Mastodon server, and it's much cheaper and easier than you might think.
If you don't want to do the techy stuff, you can use a managed hosting provider who does all the techy stuff for you (installation, upgrades, fixes etc) as well as hosting your server. These typically cost about 5 to 10 euros/dollars per month for the smallest servers.
Here's a step-by-step guide covering the whole process:

Something that could make Mastodon safer and more pleasant to use would be reply controls.
This would optionally allow users to specify who can reply to their posts, so that people who experience abusive replies from bigots, trolls etc can pre-emptively prevent that happening.
It would also give people a middle option between follower-only posts and public posts.
If you're comfortable using Github, you can vote for this by giving a thumbs up at https://github.com/mastodon/mastodon/issues/14762
